First Time Patients - How to Book
If this is your first time at Quantum Touch Acupuncture & Wellness, and you are planing on booking an acupuncture treatment, please book "Initial Quantum Acupuncture" or "Initial Energy Acu Detox." This will allow us to take your vitals and detailed health history, discuss your treatment plan, give lifestyle recommendations and dietary suggestions, and answer any of your questions.
First Time Patients - Patient Forms
Please scroll to the bottom of this page to "Patient Forms" and print, fill out, and bring them with you to your appointment. If you are not going to fill out forms at home, please come to your appointment at least 20 minutes early to do so in the waiting area. Patient forms will be attached to a clipboard in a small portable file storage.
Appointment Time
Please allow yourself enough time before the treatment, taking in consideration unexpected traffic and parking time. There is free parking on the side of the building. If the parking lot is full, street parking should be available in one of the neighboring streets.
